Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Health Care Specialist. As a Home Health Nurse, you will provide high-quality care to patients in their homes, schools, or community settings, working collaboratively with families and interdisciplinary teams to ensure the best possible outcomes.

Responsibilities
  • Provide skilled nursing care to patients with complex medical conditions, including cerebral palsy, tracheostomies, and feeding challenges
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Administer medications, perform wound care, and provide other nursing interventions as needed
  • Document patient progress and communicate effectively with healthcare teams and families
Requirements
  • Current RN licensure in the state of Massachusetts
  • Experience in home health care or pediatric/adult care
  • Strong clinical skills and critical thinking abilities
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
