Thom Child and Family Services is seeking a highly skilled and compassionate Pediatric Registered Nurse to join our team. As an Early Intervention RN, you will play a vital role in providing home and community-based services to infants and toddlers who have or are at risk for developmental delays and their families.
Key Responsibilities- Initial Intakes and Referrals: Assist and/or complete initial intakes and provide follow-up referrals and/or interagency service coordination as needed.
- Multidisciplinary Team Assessments: Participate in multidisciplinary team assessments to determine the developmental needs of the child and the concerns and priorities of the family.
- Individual Family Service Plans (IFSP): Participate in the development of IFSPs with families and collaborate with families to identify strengths and resources.
- Coaching Model Services: Provide services using a coaching model with children and families according to their IFSP, including home visits, child groups, and parent groups.
- Developmental Milestones and Progress: Monitor developmental milestones and progress with parents/caregivers to inform the ongoing need for direct service, consultation, and/or assessment.
- Parent/Caregiver Education: Provide parent/caregiver education on health and safety issues during home visits or parent education groups.
- Documentation and Reporting: Complete progress notes, reports, assessments, and correspondence as appropriate.
- Current Registered Nurse Licensure: Current Registered Nurse licensure in the state of Massachusetts.
- Bilingual/Bicultural: Bilingual/Bicultural a must.
- Strong Communication and Organizational Skills: Strong ability to engage in meaningful, therapeutic relationships with children, parents/caregivers, and community partners.
- Knowledge of Family Systems and Early Childhood Development: Knowledge of family systems and early childhood development.
- Commitment to Delivering Individualized and Culturally Informed Services: Commitment to delivering individualized and culturally informed services.
- Excellent Computer Skills: Good computer skills as well as strong documentation skills.
