The Quality Improvement (QI) Registered Nurse is tasked with developing, implementing, and evaluating all facets of quality assurance and enhancement protocols. This role is crucial in ensuring that patients receive optimal care that fosters positive health outcomes while adhering to CMS guidelines.
- Ensure compliance with Federal and State regulations and adhere to relevant accreditation standards in all Quality Assurance and Improvement initiatives.
- Uphold compliance in alignment with organizational policies and procedures.
- Facilitate the creation of patient-centered care plans from admission through discharge.
- Oversee timely requests for authorizations.
- Assess the quality and efficacy of nursing practices and services, analyzing relevant data to identify collaborative opportunities with stakeholders to enhance services and patient outcomes.
- Participate in company committees as appointed by agency leadership.
- Support the orientation, training, and education of clinical field personnel.
- Maintain and evaluate all components of Quality Assurance and Performance Improvement (QAPI).
- Monitor Electronic Medical Records (EMR) Reports related to OASIS, Plan of Care (POC), and RAP timeliness, assisting managers in developing strategies to meet deadlines.
- Act as a resource for staff regarding performance improvement across various areas, including the accuracy of information, supporting documentation, OASIS assessments, plan of care development, and overall job performance.
- Export OASIS assessments for designated branch locations weekly and as necessary, maintaining Validation Reports of OASIS transmissions in the Teams folder. Identify error patterns, staff performance issues, and recommend corrective actions and educational needs.
- Base decisions and actions on ethical principles, fostering a non-judgmental and inclusive environment sensitive to socio-cultural diversity.
- Participate in on-call duties for after-hours client care.
- Promote a culture of quality and safe client care through active participation, development, and adherence to the Quality Assurance plan and associated metrics.
- Perform additional duties and accept responsibilities as required.
- Current RN license.
- At least five (5) years of experience in home health care.
- Comprehensive knowledge of Federal and State regulations.
- Strong organizational and communication skills.
- Minimum of 1-2 years of experience in OASIS and home health documentation review.
- Willingness to travel approximately 25% of the time.
