Job Summary:
We are seeking a skilled nursing professional to join our team as a home health care specialist. As a medical care provider, you will be responsible for providing high-quality, patient-centered care to our clients in their homes or in facilities we work with.
Responsibilities:
- Assessment and Care Planning: Conduct thorough assessments of patients to identify their care needs and develop personalized care plans to meet those needs.
- Medication Management: Safely and effectively administer medications, tests, and treatments as ordered by physicians.
- Patient Education: Teach patients and their families about their conditions, medications, and self-care skills to promote independence and health.
- Documentation: Maintain accurate and detailed records of patient interactions, including medical information, treatment plans, and progress notes.
- Communication: Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Compassionate Care: Provide emotional support and comfort to patients and their families, addressing their physical, emotional, and spiritual needs.
Requirements:
- Current Massachusetts RN or LPN license in good standing
- Current Massachusetts driver's license and reliable transportation
- 1 + year of nursing experience preferred, but not required. VNA experience a plus.
- Excellent organizational and interpersonal skills
- Experience with documenting the electronic medical record strongly preferred
- Must be a fluent communicator (English); bilingual is a plus. Excellent clinical skills, bedside manner, infection control, nursing skills, physiological knowledge, administering medications, medical teamwork, multi-tasking, listening, verbal communication, health promotion, and maintenance.
- Must be able to pass a criminal background check
- Must feel comfortable seeing our clients in their homes and or facilities
