About the Role

As a Care Coordinator at OhioGuidestone, you will play a vital role in providing comprehensive care to individuals in need. Your primary responsibility will be to serve as a single point of contact for clients, guiding them through the care planning process and ensuring they receive the necessary support to achieve their goals.

Key Responsibilities
  • Coordinate Care Services: Work closely with internal and external providers to ensure a holistic approach to care coordination, addressing the physical and mental health needs of clients.
  • Client Support: Encourage client participation and screen for any barriers to treatment, working with cross-functional team members to eliminate obstacles and ensure seamless care delivery.
  • Relationship Building: Develop and maintain collaborative working relationships with internal and external referral resources, fostering a network of support for clients.
  • Scheduling and Documentation: Assist with scheduling and completing appointments, ensuring timely documentation of services in the electronic health record.
  • Community Recovery Supports: Assist in the development and identification of comprehensive community recovery supports, including peer support, counselor services, and family supports.
  • Leadership and Collaboration: Demonstrate positive leadership, promote an inclusive work environment, and present the Agency in the most positive light with all internal and external contacts.
Requirements
  • Education and Experience: Associates Degree in a related human services field or equivalent experience in a behavioral and/or healthcare setting.
  • Knowledge and Skills: Strong knowledge base of community providers and resources, effective interpersonal skills, and understanding of social drivers of health.
  • Travel and Transportation: Ability to travel to assigned sites and/or transport clients as requested or required.
