Job Summary

Key Responsibilities
  • Provide personalized care to residents, adhering to state guidelines and our organization's policies and procedures.
  • Complete administrative tasks, including documentation and charting, to ensure accurate and timely care.
  • Assist residents during admissions, transfers, and discharges, providing a smooth transition and ensuring continuity of care.
  • Follow care plans to meet the unique needs of each resident, promoting independence and dignity.
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
Requirements
  • Current State of Ohio STNA certification.
  • 1-2 years of experience in long-term care, preferably in a similar role.
  • Familiarity with basic computer skills and ability to learn new software applications.
  • Excellent communication and interpersonal skills, with a passion for delivering compassionate care.
