Position Overview

The Mission Neighborhood Health Center (MNHC) is a dedicated non-profit organization operating multiple health service sites in the vibrant communities of San Francisco. We are currently seeking qualified individuals to fill the role of Registered Nurse at our Mission Bernal Clinic. Please note: proficiency in both English and Spanish is a requirement for this position. This role reports directly to the Senior Nurse Manager and is classified as a Regular, Full-Time, Exempt position, offering a starting annual salary range of $91,000 to $97,000 along with comprehensive benefits. Newly graduated RNs are encouraged to apply.

Key Responsibilities:

  1. Deliver patient care services within the scope of nursing licensure.
  2. Conduct thorough triage and provide advice (via phone and in-person) under the guidance of licensed clinical staff.
  3. Perform comprehensive assessments of patients' physical, behavioral, and psychosocial needs.
  4. Employ behavioral strategies to assist patients and their families in adopting healthier lifestyles and enhancing self-care practices.
  5. Manage medication refills and prior authorizations in accordance with established protocols.
  6. Respond promptly to triage requests (both phone and in-person) and participate in emergency codes within the facility.
  7. Educate patients and their families about the significance of lab and radiology results as directed.
  8. Conduct Nurse Co-Visits alongside the Care Team efficiently and effectively.
  9. Develop individualized, patient-centered Care Plans.
  10. Initiate and engage in Care Conferences.
  11. Provide clinical oversight to assigned Medical Assistants.
  12. Participate in team huddles, training sessions, and meetings as requested by the Supervisor.
  13. Contribute to quality improvement initiatives.
  14. Assist in the maintenance and inventory management of medical emergency equipment and sterilization supplies.
  15. Perform additional duties as assigned.

Qualifications:

  1. Possession of a valid, unrestricted Registered Nurse license in the State of California is required.
  2. Graduation from an accredited School of Nursing is required; a two-year nursing degree is mandatory, while a Bachelor’s or Master’s degree is preferred.
  3. Current CPR and BLS certification is required.
  4. Bilingual proficiency in English and Spanish is essential.
  5. Experience with electronic medical records and practice management systems is preferred.
  6. Familiarity with population health systems is advantageous.
