Job Summary:

The Patient Support Coordinator role at Navian Hawaii is a vital position that requires a compassionate and organized individual to provide exceptional support to patients and their families. As a key member of our team, you will be responsible for ensuring seamless transitions and delivering comfort and support to those in need.

Key Responsibilities:

  • Respond to referrals promptly, gathering necessary information and communicating effectively with patients and referral sources.
  • Maintain accurate and up-to-date records of patient information and required documentation in our electronic medical record system.
  • Develop and maintain relationships with referral sources, hospital discharge planners, physicians, and community partners to ensure smooth referrals and admissions.
  • Assist the admissions team in gathering necessary paperwork and resolving problems related to insurance verification, CTI, and Start of Care Orders.
  • Log data into weekly reports and track progress against goals, sharing insights with the team to drive continuous improvement.

Requirements:

  • High school diploma and minimum one year of experience in healthcare or a related field.
  • Strong communication and customer service skills, with the ability to establish and maintain effective working relationships.
  • Excellent organizational skills, with the ability to prioritize, problem-solve, and manage multiple tasks simultaneously.
  • Familiarity with concepts and practices used in a hospice setting, including end-of-life care.
