Job Summary

The Registered Nurse Assessment Coordinator (RNAC) plays a vital role in the coordination of patient/resident care at Devlin Manor Nursing & Rehabilitation Center. Under the direction of the Director of Nursing, the RNAC is responsible for ensuring seamless care transitions from admission through discharge.

Key Responsibilities
  • Identify and monitor resident skilled services to ensure optimal care outcomes.
  • Coordinate the Resident Assessment Instrument (RAI) process to gather comprehensive data on patient/resident needs.
  • Develop and implement accurate Minimum Data Set (MDS) assessments and interdisciplinary Plans of Care.
  • Monitor patient/resident status to identify changes in condition, barriers to discharge, and progress toward care goals.
  • Participate in daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meetings to ensure collaborative care planning.
  • Establish and maintain positive relationships with healthcare team members, functioning as part of the nursing management team.
Requirements
  • Current, unencumbered RN license in the state of practice.
  • Minimum 2 years of direct hands-on nursing experience.
  • Minimum 2 years of long-term care experience.
  • Preferably 1 year of RAI experience.
  • Proficiency in ICD 10 coding required.
