Family Support Practitioner

We are seeking a highly skilled and compassionate Family Support Practitioner to join our team at Child & Family. As a key member of our team, you will play a vital role in providing essential support services to families in need.
Key Responsibilities- Deliver home-based therapy services to clients and their families, focusing on building strong relationships and promoting positive outcomes.
- Collaborate with colleagues to develop comprehensive treatment plans tailored to each family's unique needs.
- Provide education and support to family members and caregivers, empowering them to make informed decisions and take an active role in the recovery process.
- Maintain accurate records and documentation, ensuring seamless communication between team members and stakeholders.
To be successful in this role, you will require:
- A Master's degree in Social Work or a related field.
- A minimum of 3 years' experience in a similar role, preferably in a trauma-informed treatment setting.
- Strong organizational, critical thinking, and time-management skills.
- Proficiency in Microsoft Office 365 and ability to learn new software quickly.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
