About the Role:
We are seeking a highly skilled Clinical Pharmacist Specialist to join our team at UofL Health. As a Clinical Pharmacist Specialist, you will play a critical role in providing specialized pharmaceutical services to improve drug usage and therapeutic outcomes for patients.
Key Responsibilities:
- Participate in multidisciplinary patient care rounds to ensure optimal medication therapy outcomes.
- Evaluate and verify medication orders to ensure appropriateness and effectiveness.
- Provide clinical services, including medication histories, pharmacokinetic dosing, and patient counseling.
- Collaborate with pharmacy leadership to develop and implement clinical pharmacy services.
- Ensure compliance with policies and procedures at UofL Hospital.
Qualifications:
- Doctor of Pharmacy degree from an accredited College of Pharmacy.
- Post-Graduate Year 2 (PGY-2) Specialty Residency or equivalent experience in specialty area.
- At least three years of practice experience in the area of specialty.
- Leadership experience in planning and setting direction for a department, team, or program.
