Opportunity Overview

The Patient Care Coordinator role at Optimus Health Care, Inc. is a key part of our patient-centered medical home (PCMH) care team. This position supports providers, nurses, and other front office staff in ensuring the complete and timely patient flow process.

Responsibilities:

  • Coordinates patient flow and ensures efficient use of resources.
  • Administers health questionnaires to patients and updates patient information in the electronic medical record system.
  • Assists with scheduling of tests and treatments and maintains infection control and safety guidelines.
  • Explores community resources available to meet patient needs and performs other related duties as needed.

Requirements:

  • Graduated from an accredited Medical Assistant Program.
  • Professional Medical Assistant Certification obtained through one of the following agencies.
  • Experience in a clinical setting and familiarity working with Electronic Medical Records (EMR) systems.
  • Bilingual: Spanish-English or English/Creole helpful; demonstrates ability to effectively interpret medical terminology.
  • CPR certification required upon hire and recertification bi-annually.
