Job Summary

Kinetic Medical Group seeks an experienced Primary Care Nurse Practitioner to join our team at a skilled nursing facility in Cambridge, MD. As a key member of our interdisciplinary team, you will provide high-quality patient-centered care, focusing on the medical needs and overall well-being of our residents.

Key Responsibilities:
  • Conduct comprehensive assessments and evaluations of residents, identifying areas for improvement and developing individualized care plans in collaboration with the healthcare team.
  • Manage acute and chronic medical conditions, prioritizing resident safety and quality of life.
  • Perform routine physical exams and health screenings, prescribing medications and treatments as necessary to ensure optimal resident outcomes.
  • Educate residents and families on health management and wellness strategies, promoting self-care and independence.
  • Collaborate with our interdisciplinary teams to enhance care delivery, addressing complex issues and leveraging expertise to improve resident experiences.
Qualifications:
  • A minimum of 2 years' experience in primary care, geriatrics, or skilled nursing, with a proven track record of delivering exceptional patient care.
  • Strong clinical skills, with the ability to work independently and prioritize tasks effectively.
  • Excellent communication and interpersonal skills, with a focus on building strong relationships with residents, families, and healthcare colleagues.
  • A commitment to providing compassionate, patient-centered care, with a passion for delivering high-quality services that meet the unique needs of each resident.
Compensation and Benefits:
