The salary for this position is estimated to be around $85,000 - $110,000 per year, depending on experience and qualifications.

Job Description:

You will be responsible for providing high-quality, direct patient care in a team-based model to our patients. You will work closely with our interdisciplinary team to coordinate patient-centered care, support transitions between levels of care, and counsel, educate, and support patients in improving their health status.

Your key responsibilities will include:

  1. Coordinating and facilitating quality clinical management of a broad-based outpatient population across the continuum of care.
  2. Collaborating with providers to identify needs for preventive care, evidence-based chronic disease management, and care coordination.
  3. Providing general and specific healthcare education to patients.
  4. Working directly with patients to identify and monitor self-management goals.
  5. Collaborating care with community-based providers, following up on ED visits and inpatient discharges, and assisting providers with exams, treatments, and procedures as needed.
  6. Providing skilled telephonic triage and assessment, education, collaboration, and coordination of healthcare and community resources.
  7. Performing a direct patient care role, including annual wellness visits (AWVs) and chronic care management (CCM) of our patients.
  8. Administering vaccines and medications by parenteral, oral, rectal, or nasal route by orders of the prescribing provider and by the medication administration policy guidelines established by Mt. Auburn Hospital.
  9. Monitoring and ensuring follow-up of all abnormal test and imaging results under the direction of the provider.
  10. Overseeing the inventory and ordering management of clinical supplies and equipment as necessary.
Requirements:
  • Graduate from an accredited nursing program required; Bachelor's degree preferred.
  • License Registered Nurse required, or Registration Licensed Practical Nurse required, and Certificate 1 Basic Life Support required.
  • 1-3 years related work experience required.
  • CPR Certification.
  • Strong interpersonal skills, strong patient communication and patient education skills.
  • Advanced skills with Microsoft applications, including Outlook, Word, Excel, PowerPoint, or Access, and other web-based applications.

To succeed in this role, you must possess excellent decision-making, problem-solving, and teamwork skills. You should also have strong written and oral communication skills and be able to demonstrate full working knowledge of standard concepts, practices, procedures, and policies with the ability to use them in varied situations.
