Job Summary

Cape Cod Healthcare is seeking a highly skilled Social Worker MSW to join our team as a Patient Care Specialist. This role is responsible for providing discharge planning and counseling services to patients and their families, recognizing age-specific differences in assessing patient needs.

Key Responsibilities
  • Provide courteous and professional services to patients, visitors, physicians, volunteers, and other employees.
  • Screen admissions to determine whether discharge planning services are needed and assess patients who fall into high-risk categories.
  • Assess and refer outpatients as required and provide therapeutic short-term counseling to patients and families.
  • Coordinate discharge planning with patients, families, physicians, staff, and community agencies, recognizing each patient's requirements relative to their age-specific needs.
  • Determine the type and amount of home care services required by patients and arrange for their care.
  • Communicate diagnoses and activities of daily living status to nursing home staff and coordinate and document weekly multi-disciplinary rounds to ensure patients' continuing care needs are met.
  • Provide mental health and substance abuse assessments of hospital patients, recognizing each patient's requirements relative to their age-specific needs, and work with psychiatrists in assessing and treating psychiatric patients.
  • Assist in the coordination of child abuse and elder abuse procedures and assist physicians and hospital staff with their mandated reporting obligations.
  • Arrange foster care or adoptions for newborns of maternity patients who choose this option and coordinate and lead patient-focused conferences with physicians, nurses, and other staff when needed.
  • Provide social work support and education for patients and families through special projects and provide crisis intervention during covered hours.
  • Assist patients and families in filing Medicaid applications with the Department of Public Welfare and inform patients of Medicaid eligibility regulations.
  • Deliver and explain to patients and families the Medicare termination of eligibility letter and coordinate the flow of information between the hospital and other facilities and agencies.
  • Prepare and obtain patient or family signature on the written discharge plan provided to all who receive aftercare services as required by the DPH.
  • Telephone follow-up of selected discharge patients as required by the DPH and maintain social history and current progress notes in patient medical records.
  • Function as liaison and maintain strong working relationships with community agencies, including extended care facilities, mental health, and home care agencies.
  • Serve on the Psychiatric patient care committee and attend other hospital and community meetings.
  • Provide training and supervision, as appropriate, for social work students as part of their field instruction.
  • Participate in the development of in-service programs for hospital personnel addressing the psychosocial needs of patients and families.
  • Prepare and submit monthly statistical and quality assurance reports and assist in fulfilling audits and other federal, state, and JCAHO requirements.
  • Assist in assuring the utilization of acute hospital beds and participate in appropriate conferences, workshops, and courses to enhance and stimulate professional awareness of innovative medical social work programs and services.
Qualifications
  • Master's Degree in Social Work
  • Licensed in MA
  • Over three months up to and including one year of social work experience
  • Completion of an approved de-escalation Certification Course within 90 days from hire
