Position Overview:
As an Adult Services Guardian Case Specialist, you will engage in professional social work aimed at supporting eligible adults, primarily focusing on public guardianship and conservatorship.

Key Responsibilities:
  • Facilitate the delivery of essential services such as housing, employment, and community integration.
  • Engage in comprehensive care planning, including documentation and plan formulation.
  • Manage case activities for adults under guardianship, addressing medical, psychiatric, and financial decision-making.

Your Duties Will Include:
  • Advocating for clients by reviewing and documenting personalized care plans.
  • Conducting regular client visits to assess needs and ensure safety.
  • Maintaining detailed case records, including assessments and reports.
  • Collaborating with treatment teams to discuss client progress.
  • Evaluating service impacts and making informed decisions regarding service modifications.
  • Working with Adult Protective Services as needed to address emergency situations.
  • Acting in the best interests of clients as a Public Guardian and Conservator.

Qualifications:
Candidates must possess a Bachelor's Degree in Social Work or a related field with a focus on behavioral or social sciences. Eligibility for licensure as a Licensed Social Worker (LSW) is required.

Experience:
A minimum of one year of experience working with incapacitated and dependent adults is preferred. Please detail your relevant experience in your application.
