Key Responsibilities:

  • Collaborate with medical and psychiatric providers as part of the integrated care model to provide person-centered care to individuals with diverse, complex needs.
  • Develop and maintain relationships with external community partners and case managers to ensure comprehensive care.
  • Work in a fast-paced, non-traditional outpatient setting to deliver exceptional care to individuals in active substance use, requiring interpreter services, and experiencing homelessness.
