Position Overview

NORTHSTAR BEHAVIORAL HEALTH NETWORK LLC is dedicated to providing comprehensive support for individuals facing challenges related to opioid dependency. Our multidisciplinary team is committed to delivering personalized care through both individual and group interventions. Key Performance Indicators:

  • Timely communication with external stakeholders (maximum response time of two business days)
  • Ensure documentation meets 100% compliance standards
  • Maintain a professional and organized workspace (adhering to HIPAA regulations)

Core Responsibilities: (Essential Functions)

  1. Implement interventions aligned with the objectives outlined in the treatment plan.
  2. Accurately document all client interactions in the designated system, including activity notes and critical information.
  3. Engage in weekly team meetings and supervision sessions.
  4. Provide updates to courts, correctional departments, and other referral sources as needed.
  5. Collaborate effectively with external organizations.
  6. Demonstrate expertise in working with co-occurring disorders through ongoing education and practical application of skills.
  7. Complete additional tasks as directed by the supervisor.
  8. Coordinate intake processes for outpatient services, sober living arrangements, and recommended care pathways.

Facilitate referrals to substance use disorder services as indicated by the client's healthcare provider, comprehensive assessments, or treatment plans:

  1. Assist in connecting clients to mental health services as identified in assessments or treatment plans.
  2. Support clients in accessing economic assistance, social services, housing options, and prenatal care based on their individual needs.
  3. Advocate for life skills development and assist clients in accessing follow-up treatment, disease management, and educational resources, including long-term service linkages as necessary.
  4. Document the provision of treatment coordination services in the client's records.
  5. Receive weekly supervision from a licensed counselor or qualified supervisor.
  6. Other responsibilities may be assigned as needed.

Qualifications:

Required Skills and Experience: (Minimum qualifications regarding education, experience, licenses/certifications, or other relevant knowledge and skills)

  • Completion of 30 hours of classroom training in treatment coordination for individuals with substance use disorders, or willingness to complete training upon hire.
  • Possession of either:
    • (i) a bachelor's degree in behavioral sciences or a related field; or
    • (ii) current certification as an alcohol and drug counselor, level I, from a recognized authority.
  • A minimum of 2,000 hours of supervised experience working with individuals with substance use disorders.
  • Ability to pass a background check.
  • First Aid/CPR certification (or ability to obtain within 30 days of hire).
  • A minimum of 2 years of sobriety.
  • Valid driver's license and a satisfactory driving record.
  • Physical capability to perform the duties required by the position.

Preferred Skills and Experience: (Additional qualifications that are beneficial)

  • Proficient in identifying and assessing a diverse range of client needs.
  • Familiarity with local community resources and their application for client benefit.
  • Experience supervising clinical staff.
  • Knowledge of the Medicated Assisted Treatment (MAT) model.
  • Experience with Procentive or similar Electronic Health Record systems.
  • A genuine passion for assisting individuals in need and thriving in a dynamic environment.
  • Ability to manage multiple tasks effectively.
  • Dependable, organized, and professional, with the ability to work independently.
