Job Summary

The Clinical Liaison Nurse is a key member of the Healthmap Solutions team, responsible for developing and maintaining long-term relationships with healthcare providers and patients. This role requires strong communication and interpersonal skills, as well as the ability to work in a fast-paced environment.

Key Responsibilities
  • Relationship Development: Establish and maintain strong relationships with healthcare providers, including physicians, office staff, and patients.
  • Care Coordination: Coordinate care management activities to improve health outcomes for patients.
  • Performance Improvement: Identify opportunities to improve performance and implement strategies to achieve goals.
  • Education and Communication: Educate patients and healthcare providers on best practices, process improvements, and health management interventions.
  • Advocacy: Serve as a patient advocate, utilizing community resources and programs to support individualized goals.
  • Reporting and Documentation: Ensure timely and accurate reporting to internal and external stakeholders, and maintain thorough documentation of all interactions.
  • Compliance: Ensure adherence to Healthmap Solutions policies and procedures, as well as federal, state, and local regulations.
Requirements
  • Education: Bachelor's degree in Nursing required.
  • Licensure: Active, unrestricted RN license required.
  • Experience: 3+ years of progressive experience in healthcare services, clinical operations, quality, or care management.
  • Skills: Excellent verbal and written communication skills, interpersonal skills, ability to multitask and prioritize, and strong critical thinking and analytical skills.
Working Conditions

This role requires moderate travel, up to 25%, within the Central/South DE area.
