This position is responsible for managing and coordinating the inpatient care transitions program, ensuring patients progress through the care continuum in a manner that achieves designed clinical and financial outcomes.
Key Responsibilities- Develop and implement effective care transition strategies to improve patient outcomes and reduce readmissions.
- Collaborate with multidisciplinary teams, including ambulatory, post-acute providers, and health plans, to ensure comprehensive care coordination.
- Manage departmental budgets and ensure compliance with federal, state, and regulatory requirements.
- Provide mentorship and guidance to staff, promoting a culture of excellence and customer service.
- Stay up-to-date with industry trends and best practices, applying knowledge to improve care transition processes.
- Bachelor's degree in Nursing (BSN) or related field.
- Current RN licensure in North Carolina or Virginia.
- Basic Life Support (BLS) certification.
- Specialty certification in case management or a related field (e.g., ACM, CCM, CCCTM, CMAC, or CMGT-BC).
- Master's degree in Nursing or related field.
- Leadership experience in a healthcare setting.
