Job Summary

As a key member of the Sentara team, the Inpatient Care Transition Program Manager, RN, plays a vital role in ensuring seamless patient care transitions. This position is responsible for managing and coordinating the inpatient care transitions program, ensuring patients progress through the care continuum in a manner that achieves designed clinical and financial outcomes.

Key Responsibilities
  • Develop and implement effective care transition strategies to improve patient outcomes and reduce readmissions.
  • Collaborate with multidisciplinary teams, including ambulatory, post-acute providers, and health plans, to ensure comprehensive care coordination.
  • Manage departmental budgets and ensure compliance with federal, state, and regulatory requirements.
  • Provide mentorship and guidance to staff, promoting a culture of excellence and customer service.
  • Stay up-to-date with industry trends and best practices, applying knowledge to improve care transition processes.
Requirements
  • Bachelor's degree in Nursing (BSN) or related field.
  • Current RN licensure in North Carolina or Virginia.
  • Basic Life Support (BLS) certification.
  • Specialty certification in case management or a related field (e.g., ACM, CCM, CCCTM, CMAC, or CMGT-BC).
Preferred Qualifications
  • Master's degree in Nursing or related field.
  • Leadership experience in a healthcare setting.
