We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at AdventHealth Hendersonville. This individual will be responsible for coordinating patient care, collaborating with healthcare teams, and ensuring seamless transitions between care settings.

Estimated Salary: $65,000 - $85,000 per year

About the Role:

The Clinical Care Coordinator will plan, coordinate, and provide care for a geographically assigned caseload of patients. They will work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans that meet each patient's specific needs.

This role requires strong communication and organizational skills, as well as the ability to work effectively in a fast-paced environment. The ideal candidate will have a passion for delivering high-quality patient care and a commitment to excellence.

Responsibilities:
  • Initiate and maintain ongoing utilization review based on knowledge of integral screening criteria for all payers
  • Participate in interdisciplinary patient care meetings to ensure comprehensive care planning
  • Verbally communicate and provide frequent feedback to physicians regarding patient discharge needs, resources, and criteria
  • Assess, plan, evaluate, implement, and document discharge planning needs based on current knowledge of available resources
  • Maintain ongoing, collaborative communication with nursing regarding discharge plans
  • Work with other members of the Care Management Department to maintain an accurate and up-to-date list of available resources within the community
Requirements:

To be successful in this role, you will need:

  • Graduate of an accredited school of nursing
  • Current state licensure as a Registered Nurse
  • Experience as an RN commensurate with five years of acute care experience within the last ten years or five years of experience in one or a combination of home healthcare, community health, or public health, or outpatient facility experience
  • Able to communicate effectively in English, both verbally and in writing
