Patient Care Coordinator

The Clinical Nursing Assistant is a vital member of our healthcare team, responsible for delivering exceptional patient care under the direction of a Registered Nurse in a hospital setting. This role requires a high level of competence, attention to detail, and a commitment to providing compassionate care.
We are seeking a skilled and dedicated individual to join our team as a Patient Care Coordinator. In this role, you will be responsible for coordinating and delivering patient care, utilizing the nursing process to ensure the highest level of quality and safety.
Your primary responsibilities will include:
- Assisting physicians and other healthcare providers with routine examinations and procedures
- Observing, collecting, and documenting patient information obtained while providing care
- Providing care and treatments to patients as outlined in the plan of care
- Assisting patients with routine activities of daily living (ADL)
- Maintaining an effective clinical work environment
