About the Role

This is an exciting opportunity for a motivated and organized professional to provide acute and longitudinal patient care. As a member of our interdisciplinary team, you will work collaboratively with medical providers, care management, and other clinical staff to enhance the quality of patient management and satisfaction.

Key Responsibilities
  • Assess and document bio-psychosocial needs, developing formal intervention plans to support patient success.
  • Evaluate and identify potential or actual patient/family crises, providing crisis intervention and emotional support, and creating effective plans for resolution.
  • Deliver information and short-term therapeutic counseling to patients and their families, addressing social, psychological, and family situations.
  • Maintain relationships with community health and social agencies, monitoring resources to meet patient needs.
  • Refer patients and families to appropriate community assistance resources.
Requirements
  • Licensed Clinical Social Worker (LCSW) certification
  • Master's degree in Social Work (MSW) from a Council on Social Work Education-accredited program
  • BLS certification
