Job Summary

North Suffolk Mental Health Association is seeking a Senior Community Support Specialist to provide direct services to individuals in community and group living environments. The ideal candidate will have a strong background in human services and be able to work collaboratively with a dedicated team to provide interventions and support to individuals with severe and persistent mental illness and/or substance use disorder.

Key Responsibilities
  • Engage individuals and partner with them to improve their health and wellness, achieve symptom stabilization and self-management, and develop and/or restore skills impacted by mental illness.
  • Gather information about community agencies to assist in the provision of services.
  • Organize and lead individual and group social and recreational activities.
  • Carry out assigned interventions and action steps of the treatment plan through the provision of direct support and rehabilitation services to individuals on an individual, group, and family basis.
  • Collaborate with Housing Coordinators to help individuals find safe and affordable housing.
  • Participate in medication administration per MAP and DMH policies and protocol.
  • Help individuals access reliable transportation.
  • Assist with supervision of Community Support Specialists.
Requirements
  • High School Diploma/GED required.
  • Bachelor's degree required. Master's or Doctoral degrees in human services-related field preferred.
  • At least two years of experience working with individuals with severe and persistent mental illness and/or substance use disorder, or established significant expertise in specialized areas related to ACCS components of care.
  • Must have a Driver's license.
  • Must be MAP and CPR/First Aid certified within five months of hire.
