Essential Duties:
Patient Outreach:
Conduct outbound calls to individuals responsible for long-term care residents eligible for the CCM program, explaining benefits, addressing inquiries, and obtaining consent for enrollment.
Program Overview:
Provide clear and concise explanations of the CCM program, detailing services offered, potential costs, and benefits of participation.
Documentation:
Maintain accurate records of all interactions, consents obtained, and any necessary follow-up in both spreadsheet and EMR formats.
Regular Reporting:
Deliver consistent updates to management on patient interactions and program advancements.
Trend Analysis:
Regularly report on the number of consents secured, refusals, and any patterns observed in patient interactions.
Flexible Availability:
Occasionally work during weekends or off-hours as needed, accounting for less than 20% of total hours.

Shift Structure:
Monday-Friday, 8-hour shifts (Typically 9 am-5 pm but may vary)
Qualifications:
Minimum of a High School diploma or GED is required.
Preferred: Associate's or Bachelor's degree in a healthcare-related field, or 1-3 years of relevant experience.
Strong communication and interpersonal skills.
Ability to simplify complex medical programs for better understanding.
Proficiency in EHR systems, Excel spreadsheets, Microsoft Teams, and other relevant software.
Strong organizational skills and attention to detail.
Ability to manage sensitive patient information with confidentiality and professionalism.
Physical Requirements:
Must adhere to the company's Time and Attendance policy.
Must be able to remain seated for the majority of the shift.
