Essential Duties and Responsibilities

Oversee, monitor, and manage your own patient caseload across various facilities to:
Coordinate the execution of Chronic Care Management (CCM) services for patients with chronic illnesses in long-term care settings.
Create personalized Care Plans for individuals with chronic conditions.
Regularly update and track progress monthly.
Monitor laboratory results, vital signs, medications, and testing for your patient population.
Facilitate care coordination with patients, their healthcare teams, and families through various communication methods.
Provide education to patients regarding the management of chronic conditions, medications, and necessary testing.
Maintain constant communication and collaboration with care team members, including physicians, physician assistants, nurse practitioners, pharmacists, and other healthcare professionals.
Identify and address clinical and non-clinical gaps in patient care.
Support training and development for providers and the CCS team.

Shift Structure

Monday-Friday, 8-hour shifts (Typically 9am-5pm but may vary)

Requirements and Qualifications

RN in Applicable State
Must possess an active compact license
Reliable home internet and proficient computer skills required
A minimum of five years of nursing experience required
Strong communication and time management skills essential
Critical thinking and problem-solving abilities are necessary
Self-motivated and capable of working independently
Experience with chronically ill patients preferred
Telephonic case management or care coordination experience preferred
Compassionate and approachable demeanor

Physical Requirements

Must adhere to the company's Time and Attendance policy.
Ability to remain seated for the majority of the shift.
