Job Summary

The Behavioral Health Coordinator is a core member of the collaborative care team at University Hospitals. This role supports and coordinates the behavioral, mental, and physical health care of patients on an assigned caseload. The Behavioral Health Coordinator works closely with medical providers and other internal and community behavior and/or mental health providers to ensure comprehensive care.

Key Responsibilities
  1. Support the behavioral, mental, and physical health care of patients on an assigned caseload.
  2. Closely coordinate care with the patient's medical provider and other behavior and/or mental health providers, schools, and community agencies.
  3. Screen and assess patients for common behavioral, mental health, and substance use disorders.
  4. Facilitate patient/caregiver engagement and follow-up care.
  5. Provide caregiver and patient education about common behavioral, mental health, and substance use disorders and available treatment options.
Requirements
  • Bachelor's Degree required, Master's Degree preferred.
  • 2+ years experience in healthcare, behavioral health, or medical setting.
  • Experience with assessment and treatment planning for common behavioral/mental health and/or substance use disorders.
  • Licensed Social Worker (LSW or LISW) in the State of Ohio (preferred) or Licensed Professional Counselor (LPC or LPCC) in the State of Ohio (preferred).
