Responsibilities- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
- Document patient progress and care provided in a timely and accurate manner.
- Graduate of an accredited LPN/LVN school of nursing.
- Current Oregon Licensed Practical Nurse (LPN) license.
- One year of experience in nursing, preferably in home health or hospice care.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
