About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team in Oregon. As an LPN, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
  • Document patient progress and care provided in a timely and accurate manner.
Requirements
  • Graduate of an accredited LPN/LVN school of nursing.
  • Current Oregon Licensed Practical Nurse (LPN) license.
  • One year of experience in nursing, preferably in home health or hospice care.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
