Job Summary:
This Registered Nurse position is responsible for coordinating patient needs with the Cooperative Care Team throughout the continuum of care by an interdisciplinary approach that integrates settings, services, health care practitioners, and care levels. The position does not involve direct patient care. Scheduling will require flexibility based on patient needs and coverage for other Cooperative Care Team members; this includes weekends and holidays.
Key Responsibilities:
- Coordinate patient needs with the Cooperative Care Team
- Develop and implement care plans that meet patient needs
- Collaborate with healthcare practitioners to ensure seamless care transitions
- Provide education and support to patients and families
- Participate in quality improvement initiatives to enhance patient care
Requirements:
- RN with at least 5 years of experience in acute care nursing required
- BSN preferred
- Utilization review/discharge planning, case management experience preferred
- Current RN licensure to practice professional nursing in the Commonwealth of Kentucky
- CCM preferred
