Job Title: Intake Coordinator

At Southern Home Care Services, Inc., we are seeking a highly skilled and detail-oriented Intake Coordinator to join our team. As a key member of our healthcare team, you will be responsible for ensuring seamless patient registration and initial chart builds.

Key Responsibilities:

  • Ensure accurate and complete patient registration forms, collaborating with Regional Administrators as needed.
  • Provide education to patients and referral sources about our services and benefits.
  • Create new patient accounts, entering pertinent medical history and contact information.
  • Gather information and documents from patients, HIE feeds, or other EMR systems.
  • Build relationships through friendly and timely responses about the patient registration process, setting expectations for next steps.
  • Process incoming requests for records from patients, caregivers, payors, or healthcare professionals.
  • Process provider requests for records, including previous PCP, specialist, lab, and diagnostic results, hospital, or LTC records.
  • Ensure medical records are organized, accurate, and complete.
  • Communicate with other departments regarding referral status.
  • Maintain confidentiality of patient and proprietary information, ensuring compliance with our medical records policy and HIPAA regulations.

Requirements:

  • High School Diploma/GED or equivalent; Associate degree or some college preferred.
  • Minimum two (2) years of experience collecting referral information in the healthcare market.
  • Medical Assistant certification preferred but not required.
  • Experience working with all payer types, including Medicare, Medicaid, and commercial insurance companies.
  • Understanding of the scope of services offered by Abode Care Partners and BrightSpring sister companies.
  • Strong verbal and written communication skills.
