About the Role

We are seeking a highly skilled and organized Care Coordinator to join our team at BrightStar Care of Louisville. As a key member of our core team, you will play a vital role in connecting our caregiving and nursing workforce with the needs of our clients and enhancing the smooth operations of our facilities.

This is a full-time position that requires a strong ability to multitask, work well under pressure, and provide exceptional customer service. You will be responsible for scheduling staff to meet client and facility care needs, minimizing non-billed overtime, and ensuring on-call phone and assigned staff are properly trained and available as needed.

Responsibilities
  • Customer/client care: effectively manage customer care procedures and serve as the point of contact for client and facility relations.
  • Scheduling and care coverage: support and use all technology tools and resources to ensure proper business processes; schedule staff to meet client and facility care needs and minimize non-billed overtime.
  • Assist with the intake of new clients and inform potential clients of our services and reasons to utilize BrightStar over other agencies.
  • Assist in the development of organization goals and agency performance improvement activities.
  • Administer agency policies and procedures.
  • Manage collection of required Dr's orders and plans of care.
  • Supervise, educate, and coach other field personnel, as directed by the Director of Nursing or Care Coordinator team.
  • Provide information to be used for statistics, reports, and records for purposes of evaluation and reporting of organization activities.
  • Assure proper maintenance of clinical records in compliance with local, state, and federal laws.

Requirements

We are looking for a highly motivated and organized individual with a strong background in customer service and scheduling. The ideal candidate will have:

  • High school graduate required.
  • Proficiency in computer skills using Windows.
  • Prior scheduling experience preferred, ideally in the health care field.
  • Flexibility and good time management skills.
  • Excellent organization, planning, and project management skills.
  • Valid driver's license and an automobile required.

Drug screen and criminal background checks are conducted on all applicants.
