**About the Opportunity:**

We are excited to offer a unique opportunity for a Community Health Navigator to join our team at St. John's Community Services in Washington, D.C. This role involves navigating the healthcare system on behalf of individuals with intellectual and developmental disabilities (IDD).

**Key Responsibilities:**

  • Navigating the healthcare system to identify and coordinate services and resources for individuals with IDD.
  • Communicating effectively with healthcare professionals and support teams to ensure continuity of care.
  • Maintaining accurate and timely documentation of appointment details, diagnoses, treatment plans, and follow-up recommendations.

**Requirements:**

  • A minimum of 1 year of experience working with individuals with IDD or in a healthcare setting.
  • Strong verbal and written communication skills, with the ability to interact effectively with healthcare professionals and support teams.
  • Proficiency in using Therap or similar documentation systems.
