Job Summary

Penn Medicine is seeking a highly skilled Diabetes Nurse Practitioner to join our team in the Inpatient Setting. As a key member of our multidisciplinary team, you will provide exceptional care to patients with diabetes, working collaboratively with hospital staff to improve patient outcomes.

Responsibilities
  • Provide direct clinical care to patients with diabetes, utilizing evidence-based practice standards and established protocols.
  • Maintain accurate and confidential medical records, documenting all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations in the electronic health record (EHR).
  • Identify yourself properly to patients and families, clarifying your role as a nurse practitioner if necessary.
  • Function within your scope of practice, referring to collaborating physicians when outside of scope or directed by them.
  • Perform interventions specific to assigned patient population and collaborative agreement.
  • Participate in performance improvement processes to meet quality of care and service standards.
  • Demonstrate awareness of professional responsibility as a role model to peers and members of the healthcare team.
  • Perform approved therapeutic or diagnostic procedures based on patient clinical status.
  • Address patient and family concerns, creating a therapeutic relationship to provide support.
  • Provide patient education, counseling, and instruction regarding patient diagnosis, medical treatments, medications, and preventative care.
  • Participate in and support accreditation, compliance, and regulatory activities of the organization.
  • Precept NP students and other healthcare learners.
  • Identify patient/family needs for community resources and refer appropriately.
Requirements
  • Basic Cardiac Life Support (BLS) certification.
  • BLS/CPR certification as a healthcare provider, as per the American Heart Association (for providers in the ambulatory setting).
  • Certified Nurse Practitioner (CNP) certification.
  • Prescriptive authority.
  • Master of Arts or Science degree in Nursing or equivalent experience.
  • New to practice or < 2.5 years, the CRNP must be reviewed by the Credentials Committee and approved by the Medical Board.
