We are seeking a highly skilled Diabetes Educator to join our team at St. Christopher's Hospital for Children, a partnership between Tower Health and Drexel University. As a Diabetes Educator, you will play a critical role in providing education and counseling to clients with diabetes, collaborating with the care team to positively impact patient and organizational outcomes related to diabetes care.

Responsibilities
  • Provide education and counseling to clients with diabetes, including acute care referrals.
  • Collaborate with the care team to identify patients' learning needs and develop effective education plans.
  • Provide expert counsel to clinical staff regarding recognized Diabetes Guidelines, diabetes medications, monitoring equipment, and other technological tools for diabetes self-management.
  • Collaborate with interprofessional care teams and nursing leadership to positively impact patient and organizational outcomes related to diabetes care.
Qualifications
  • Bachelor of Science in Nursing, or Non-Nursing Bachelor's degree with an MSN, or Doctoral degree in Nursing.
  • Diabetes Educator experience.
  • BLS Certification, PA Registered Nurse License, and CDE or CDCES Certification (required within 12 months of hire).
