As a Diabetes/Endocrinology Nurse Practitioner, you will be part of a multidisciplinary team providing high-quality comprehensive care in the Ambulatory setting, focusing on patients with diabetes.

Responsibilities include:

  • Providing direct clinical care for patients according to established protocols and evidence-based practice standards
  • Maintaining accurate and confidential medical records
  • Documenting medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations in the electronic health record
  • Identifying self properly to patients and families and clarifying the role of the nurse practitioner if necessary
  • Performing interventions specific to assigned patient population and collaborative agreement
  • Participating in performance improvement processes to meet quality of care and service standards
  • Demonstrating awareness of professional responsibility as a role model to peers and members of the healthcare team
  • Performing approved therapeutic or diagnostic procedures based on patient's clinical needs
  • Providing patient education, counseling, and instruction regarding patient diagnosis, medical treatments, medications, and preventative care
  • Participating in and supporting accreditation, compliance, and regulatory activities of the NP students and other healthcare learners
  • Identifying patient/family needs for community resources and referring them

Requirements include:

  • BLS/CPR certification as a healthcare provider according to the American Heart Association
  • Board Certified as a Nurse Practitioner
  • Pennsylvania CRNP licensure
  • Pennsylvania Prescriptive Authority
  • Active Pennsylvania State Registered Nurses License
