Job Summary

Penn Medicine is dedicated to delivering exceptional patient care, advancing medical research, and educating future healthcare leaders. As a Diabetes/Endocrinology Nurse Practitioner, you will be part of a multidisciplinary team providing high-quality comprehensive care in the Ambulatory setting. Your expertise will focus on patients with diabetes, and you will be responsible for maintaining certifications and licenses, including acquiring and maintaining certification as a Certified Diabetes Care and Education Specialist.

Responsibilities
  • Provide direct clinical care for patients with diabetes, adhering to established protocols and evidence-based practice standards.
  • Maintain accurate and confidential medical records, documenting all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations in the electronic health record (EHR).
  • Identify yourself properly to patients and families, clarifying your role as a nurse practitioner if necessary.
  • Function within your scope and refer to the collaborating physician when outside of scope, or when directed by the collaborating physician or requested by the patient and/or family.
  • Participate in performance improvement processes to meet quality of care and service standards.
  • Demonstrate awareness of professional responsibility as a role model to peers and members of the healthcare team.
  • Perform approved therapeutic or diagnostic procedures based on the patient's clinical status.
  • Address patient and family concerns, creating a therapeutic relationship to provide support.
  • Provide patient education, counseling, and instruction regarding patient diagnosis, medical treatments, medications, and preventative care.
  • Participate in and support accreditation, compliance, and regulatory activities of the organization.
  • Precept NP students and other healthcare learners.
  • Identify patient/family needs for community resources and refer them appropriately.
Credentials
  • BLS/CPR, as a healthcare provider, as per the American Heart Association (for providers in the ambulatory setting) required.
  • Board Certified as a Nurse Practitioner (required).
  • Pennsylvania CRNP licensure required.
  • Pennsylvania Prescriptive Authority required.
  • Active Pennsylvania State Registered Nurses License (required).
  • PA and/or NJ RN licensure (RN) required.
Education or Equivalent Experience
  • Bachelors & Master of Science in Nursing (required).
  • New to Practice or over 2.5 years of experience.
  • The CRNP must be reviewed by the Credentials Committee and approved by the Medical Board.
