Care Management Specialist

Community Health Centers of the Rutland Region is seeking a compassionate and organized Nursing Home Care Coordinator to support our healthcare team. In this role, you will work closely with providers to manage workflow, perform administrative tasks, and provide high-quality patient care.
The successful candidate will possess a current Vermont LPN license, MA, and Valid CPR Certification. Previous experience in a nursing home setting is an asset, but not mandatory. Proficiency in electronic medical records and basic computer skills are required.
Your responsibilities will include communicating with providers, recording vital signs, updating patient information, documenting nursing home data, and maintaining regulatory compliance. You will also be responsible for addressing daily summary action reports and ensuring accurate patient records.
We offer a competitive salary range of $24.50-36.61 per hour for LPNs and $19.00-28.50 per hour for Medical Assistants.
