We are seeking a highly motivated and compassionate Patient Care Specialist to join our team at Avanta Care Home Care in Atlanta. As a Patient Care Specialist, you will play a vital role in providing personalized care and support to patients in their homes.

Job Responsibilities:
  • Daily Activities: Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles, and provide care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
  • Patient Care: Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor, and build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.
Requirements:
  • Qualifications: High School Diploma or GED, one year prior professional experience, drivers license required, and CPR certification required.
