Job Responsibilities:

Position Overview:
Deliver continuous service coordination, treatment strategy formulation, advocacy, and oversight to individuals in Milwaukee, Waukesha, Racine, and Kenosha counties engaged in the OARS Program (Opening Avenues for Re-entry Success), Conditional Release Program (CR), Outpatient Competency Restoration Program (OCRP), and/or Jail-Based Competency Restoration Program (JBCR).

Key Responsibilities:

Facilitate, strategize, and ensure adherence to community treatment for individuals involved in the OARS, CR, OCRP, and/or JBCR Programs.

Establish and nurture a supportive one-on-one therapeutic rapport with each participant.

Integrate evidence-based methodologies into service delivery, including but not limited to Motivational Interviewing (MI), Trauma-Informed Care (TIC), and Person-Centered Planning.

Employ MI techniques as a primary strategy to boost participant engagement and success.

Assess ongoing treatment requirements and compliance for all program participants by providing evaluation, treatment strategy development, assertive case management, symptom oversight, medication supervision, crisis intervention, and coordination of multidisciplinary team meetings in accordance with program protocols and the Department of Health Services (DHS) contract.

Responsible for drafting and submitting necessary court documents for CR participants, including:
Predisposition Investigation, Conditional Release Plan, Treatment Plan Adjustments, Adjustment Summaries, Discharge/Transition Plans, and all Status Report documents. Coordinate team meetings with DHS/Department of Corrections (DOC) and other treatment team members. Maintain safety protocols and continuously evaluate potential risks when in the community and while working with participants.

Facilitate community support services (referrals) and/or directly assist participants with daily living activities, including but not limited to coaching and hands-on support in housing, financial management, vocational/educational pursuits, scheduling, and transportation for appointments.

Encourage and assist each participant in building a natural support network comprising family members, neighbors, friends, and community resources.

Create and maintain participant service documentation, including but not limited to case notes, assessments, Adult Family Home/Rent Justification forms, release plans, Individual Service Plans (ISP), Quarterly Progress Notes (QPN), crisis plans, budgets, suicide risk assessments (SRA), trauma assessments, other risk assessments, and release of information (ROI) forms within designated program timelines.

Adhere to program and DHS quality standards, SharePoint protocols, formatting requirements, and timelines for all documentation and communications.

Ensure maximum participant financial contribution and third-party payment towards service costs by applying for suitable insurance benefits and fully utilizing available assistance programs, county services, and community resources.

Maintain ongoing communication and coordination with treatment providers, DOC, mental health institution personnel, court officials, prison/jail staff, and other service agencies, ensuring prompt responses to emails and phone messages within 24 hours.

Conduct frequent home visits and transport participants within the community using personal vehicles.

Develop and refine MI skills through training, coaching, and participant interactions to achieve basic fidelity within 12 months and advanced fidelity within 24 months of employment as outlined in the program contract.

Participate in MI coaching sessions and complete DHS-required surveys as necessary. Complete MI audio recordings and written Tests of Knowledge as required. Cultivate and maintain positive relationships with stakeholders and funders.

Additional Duties and Responsibilities:
Timely collection of essential medical records, lab results, and information for participants per program protocol. Rotational crisis line coverage. Appear and testify at court hearings. Attend training sessions, meetings, and staffings. Participate in new staff shadowing rotation. Other job-related tasks may be necessary to fulfill the responsibilities of the position. Remote staff may be required to work from the Forensic office in Milwaukee. On the last day of employment, staff will return all agency equipment to the supervisor/program director.

Job Qualifications:

Knowledge, Skills, and Abilities:
Familiarity with and experience in mental health and substance use services, including assessment, treatment plan development, psychotropic medications, side effects, and symptom management. Understanding of the legal system and forensic psychiatric issues. Risk management skills. Resourcefulness and adaptability in response to changing participant needs. Knowledge of entitlements such as Medicaid, Medicare, SSI/SSDI, and Veterans benefits. Strong written and verbal communication skills. Positive interpersonal skills. Effective organizational and time management abilities. Capacity to work independently as well as collaboratively within a team. Proficient computer skills. Commitment to social justice.

Minimum Qualifications:

Bachelor's Degree in a relevant human service field. Experience in case management and service coordination. Experience serving individuals with mental illness, substance use disorders, or other special populations is required. Must possess a valid driver's license and an insured, safe, operable vehicle. Must be able to pass criminal and caregiver background checks.

Work Relationship and Scope:
Reports to the immediate Program Supervisor.

Engages with a diverse range of individuals, including participants, staff, DHS, DOC, court personnel, institution staff, prison/jail staff, psychiatrists, treatment providers, peer support, family members, and the public.

Working Conditions:

Job is performed in a combination of office settings and fieldwork throughout a four-county region with individuals participating in the CR/OARS/OCRP/JBCR programs; some of whom may be unstable and symptomatic at times.

The position requires travel to various state prisons and/or mental health institutions throughout WI. Most work is conducted in the community, primarily at participants' residences. Court appearances are required for CR. Staff members are expected to transport participants as needed.

Typical Work Week:

Monday-Friday 8:30-4:30, although some flexibility is necessary to accommodate participant needs (to include possible extended workday hours, weekends, and holiday coverage).

Also involves rotational 24-hour crisis line coverage.

Physical Demands:
Frequent driving throughout a four-county region and to various prisons/jails and mental health institutions throughout WI is required. The position necessitates the ability to drive for extended periods throughout the day.

Must be capable of ascending and descending stairs, lifting and carrying up to 20 pounds to assist with routine participant moves.

Staff may be expected to assist participants with light household chores as needed. Office work involves sitting at a desk and performing computer tasks.
