- Case Management: Manage assigned caseloads, schedule home visits, and inform site leadership of coverage needs.
- Patient Advocacy: Act as a patient advocate, ensuring optimal outcomes and promoting patient-centered care.
- Professional Development: Stay current with industry developments, attend training sessions, and maintain certifications as required.
- Education: Master's Degree in Speech Pathology.
- Experience: Typically requires 3 years of experience in speech therapy within acute hospital or home health care settings.
- Licensure: Speech-Language Pathology license issued by the state of Wisconsin, and Basic Life Support (BLS) certification issued by the American Heart Association (AHA).
- Skills: Excellent communication and interpersonal skills, intermediate computer skills, and ability to work independently.
- Physical Demands: Must be able to move, lift, and support up to 50 pounds, and have the physical stamina to attend to patient care requirements over a period of three or more hours without complete rest.
- Environmental Factors: Must be able to work safely with people of various sizes and weight with diverse and complex medical conditions and physical limitations.
