Position Overview

Our comprehensive programs are designed to support patients and their families during difficult times, whether in a community setting or a healthcare facility.

As a Licensed Practical Nurse (LPN) under the supervision of a Registered Nurse (RN) Case Manager, you will be responsible for visiting patients in their residences or care facilities. You will adhere to each patient's individualized care plan, delivering direct nursing care and meticulously documenting each visit using our electronic medical record (EMR) system. It is essential to communicate any changes in the patient's health status to the RN Case Manager promptly.

Position Details:

  • Full-time overnight shifts available.
  • Must be a graduate of an accredited Licensed Practical Nurse program.
  • A minimum of one year of experience in medical-surgical nursing and one year in hospice or oncology nursing is required.
  • Proficient computer skills are essential.
  • A valid driver's license and reliable transportation are necessary for patient visits.
