Job Overview

Responsibilities
* Provide comprehensive nursing assessments and develop personalized care plans for patients and their families
* Coordinate and manage patient care services, including medication management and symptom control

* Educate patients and families on self-care techniques, disease management, and end-of-life care options
* Maintain accurate and timely documentation of patient information and care activities

Requirements
* Current RN licensure in Minnesota
* American Heart Association CPR certification
* Bachelor's degree in Nursing or related field preferred
* Minimum 1 year of hospice experience, with a focus on palliative care and community health
* Excellent communication, teamwork, and problem-solving skills
