Job Summary

We are seeking a Clinical Care Coordinator to join our team in New York State. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Salary: $85,000 - $105,000 + $15,000 Sign on Bonus, with opportunities for advancement and professional growth.

The Clinical Care Coordinator will be responsible for conducting comprehensive assessments of patients' clinical, psychological, and social needs using the Uniform Assessment System (UAS-NY). They will determine eligibility for enrollment in our programs and develop individualized plans of care.

  • Conduct regular home visits to assess patients' needs and provide ongoing support.
  • Collaborate with healthcare professionals to ensure seamless coordination of care.
  • Develop and implement individualized plans of care to meet patients' unique needs.
