Job Description

This role is perfect for individuals who want to make a real difference in people's lives. As a Care Manager, you'll develop, assess, and coordinate holistic care management activities to ensure quality, cost-effective healthcare outcomes.

Duties and Responsibilities
  1. Evaluate member needs, resources available, and recommend personalized service plans
  2. Develop ongoing long-term care plans and identify required services, providers, and community resources
  3. Coordinate between members, caregivers, and care providers to ensure accessible services
  4. Provide resource support for various needs based on service assessments and plans
  5. Monitor care plans, member status, and outcomes, providing recommendations as needed
  6. Interact with healthcare providers and partners to ensure member needs are met
  7. Collect, document, and maintain member information and care management activities
  8. Perform home and/or site visits to assess member needs and collaborate with healthcare providers and partners
  9. Provide education to members and their families/caregivers on procedures, healthcare provider instructions, service options, referrals, and benefits
  10. Offer feedback to leadership on opportunities to improve and enhance quality of care and service delivery
Requirements
  • Bachelor's degree and 2-4 years of related experience OR equivalent experience acquired through accomplishments reflective of this position
  • RN or LCSW required
  • Two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience
