RN (Per Diem/On Call) - PACE/Elderplace

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team at Elderplace in Gresham, Portland. As a Per Diem RN, you will provide high-quality nursing care to our patients in a dynamic and supportive environment.
Key Responsibilities- Provide professional, independent nursing care to patients in accordance with the Oregon Nurse Practice Act and the nursing process.
- Assess, plan, order, give, delegate, teach, and supervise professional nursing care delivery to promote patients' optimal health and independence.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
- Participate in ongoing education and training to stay current with best practices and evidence-based care.
- Oregon Registered Nurse License upon hire.
- National Provider BLS - American Heart Association within 30 days of hire.
- Preferred Qualifications: Bachelor's Degree - Nursing, 1 year - Home health RN experience.
