Job Title: Community Health Navigator

We are seeking a compassionate and empathetic Community Health Navigator to join our team at Ohana Pacific Health. As a Community Health Navigator, you will play a vital role in supporting patients with navigating and accessing health and community services.

Job Summary:

The Community Health Navigator will work closely with patients, families, and healthcare providers to identify barriers to care and develop personalized plans to improve health outcomes. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Identify and address barriers to care, including financial, transportation, insurance, and language barriers.
  • Develop and implement personalized care plans to improve health outcomes.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination.
  • Provide education and support to patients and families on health and wellness topics.
  • Facilitate remote visits between patients and providers using telephone or video conferencing.
Requirements:
  • High school diploma or equivalent.
  • Associates or Bachelors Degree in Social Work or other healthcare related field, community health worker certificate, or nurse aide training preferred.
  • Experience in healthcare or home health field; work with the geriatric population preferred.
