Job Overview:

As a Community Care Associate I, you will engage in essential case management tasks for Community Care Services Eligibility (CCSE).

Key Responsibilities:

  • Assess and determine eligibility for CCSE program services.
  • Authorize and oversee the delivery of CCSE program services.
  • Conduct interviews with elderly individuals and those with disabilities.
  • Accurately document information in various automated systems.
  • Verify case details using multiple automated resources.
  • Clearly explain program benefits and requirements to clients.
  • Make referrals to other programs and agencies as necessary.
  • Complete special assignments and prepare reports as needed.
  • Ensure confidentiality of all incoming and outgoing information.
  • Adhere to all relevant policies and procedures of HHSC and CCSE.
  • Provide exceptional customer service through various communication channels.
  • Maintain a positive and productive demeanor in a dynamic work environment.

Essential Functions:

  • Attend work consistently and predictably, following agency leave policies.
  • Perform additional duties as assigned.
  • Interview applicants and conduct home/site visits to evaluate service needs.
  • Collaborate effectively with consumers, families, provider agencies, and state staff to gather information and ensure accurate service authorization.
  • Conduct reviews to assess and re-evaluate eligibility.
  • Gather, verify, and compute income and resources to establish financial eligibility.
  • Document information in various automated systems accurately.
  • Develop and coordinate client service plans, authorizing services to meet individual needs.
  • Act as an advocate for consumers, assisting in locating resources and making appropriate referrals.
  • Prepare basic-level reports, narratives, and statistical summaries.
  • Monitor services provided by contracted agencies to evaluate effectiveness and identify issues.

Required Skills and Qualifications:

  • Proficient in operating computers and general office equipment.
  • Experience working with elderly individuals and/or persons with disabilities is preferred.
  • Strong interviewing, communication, and problem-solving skills, along with effective documentation abilities, are preferred.
