Position Overview:

The Patient Care Coordinator is responsible for overseeing the care and services provided to a designated patient population throughout their healthcare journey. This role ensures the delivery of high-quality clinical outcomes while managing costs effectively. The Coordinator will work closely with patients and their families, as well as lead a multidisciplinary team to facilitate comprehensive care.

Key Responsibilities:

1. Collaborate with physicians, healthcare providers, and the multidisciplinary team to establish patient goals, care plans, and monitor progress.

2. Develop, implement, and uphold work standards and procedures that align with organizational policies and regulatory requirements.

3. Stay informed about advancements in the field to recommend new services, products, and technologies.

4. Assess, create, implement, and evaluate a thorough care plan through a collaborative process involving the patient, family, and various agencies.

5. Continuously adjust the care plan based on ongoing assessments of the patient's condition.

6. Take responsibility for the effectiveness of the care plan and patient outcomes.

7. Evaluate the appropriateness of care levels, diagnostic tests, clinical procedures, and ensure complete documentation of medical records.

8. Review patient admission data in accordance with established criteria to determine the appropriate level of care.

9. Develop and monitor clinical pathways, addressing any variances as necessary.

10. Maintain open communication with patients, families, medical staff, caregivers, and third-party payers.

11. Assist patients and families with educational resources before admission, during their stay, and post-discharge.

12. Ensure patients understand third-party payer guidelines and facilitate discharge planning as directed by physicians.

13. Foster a positive work environment and contribute to the overall team efforts within the healthcare facility.

14. Participate in the collection and analysis of outcome data.

15. Perform additional duties as assigned.

Qualifications:

- Bachelor of Science in Nursing (BSN) preferred with a minimum of 5 years of experience in a hospital or community setting.

- Alternatively, must complete a BSN or a degree in a related field within 5 years.

- Registered Nurse (RN) with a Master of Science in Nursing (MSN) or a degree in a related field with at least 3 years of experience.

- Must hold all necessary Licensure and Certification in the state of employment.

- Certification from the Case Management Society of America is preferred.

- Exceptional oral and written communication skills are essential.

- Proficiency in applying advanced principles and techniques central to nursing is required.

- Strong organizational skills are a must.

- A valid driver's license is required, along with the ability to travel to various locations as needed.

- Cross-training in Utilization Review, Quality Assurance, discharge planning, and infection control is required.
