Job Overview

Position Title: Support Broker

Work Schedule: Standard hours are Monday to Friday, from 8:00 AM to 4:30 PM, with potential for additional hours as required.

Reporting Structure: This role reports directly to the PDS Director.

Role Summary: The Support Broker serves as the Case Manager for Participant Directed Services under the 1915c waivers. This position may be Full or Part-time and includes on-call responsibilities. Participants have the autonomy to determine their services, including scheduling, employment, budgeting, and evaluating service effectiveness. The Support Broker will provide training, technical assistance, answer inquiries, coordinate services, and community resources, monitor service delivery, and assist in creating a person-centered Plan of Care (POC), which includes an emergency backup plan and support spending plan.

Key Responsibilities:

  • Collaborate with individuals and their designated team to formulate an Individual Plan of Care (POC), budget, spending plan, and emergency backup plan.
  • Employ a person-centered planning methodology to identify and implement necessary supports.
  • Manage a maximum caseload of 40 PDS clients.
  • Assist PDS participants with various aspects of PDS or blended services, including provider recruitment, hiring, and management, as well as completing required documentation.
  • Coordinate all service provisions.
  • Monitor the health, safety, and welfare of participants.
  • Conduct monthly face-to-face home visits.
  • Maintain availability 24/7, including holidays.
  • Document all interactions with participants and track progress towards goals.
  • Address issues related to unmet needs or safety concerns and ensure proper utilization of budgeted funds.
  • Report any instances of abuse, neglect, or exploitation as mandated.
  • Perform additional duties as assigned.

Essential Skills:

  • Complete training requirements within specified timeframes as per SCL/MPW Waivers and DDID standards.
  • Adhere to agency policies, SCL/MPW regulations, and DDID standards.
  • Possess a valid driver’s license with a clean driving record.
  • Pass all background checks.
  • Have a valid Social Security number or work permit if not a US citizen.
  • Complete TB Risk Assessment within 7 days of hire.
  • Pass a drug test.
  • Understand and promote the MR/DD Vision Statement.
  • Exhibit strong oral and written communication skills.
  • Demonstrate interpersonal skills and the ability to exercise sound judgment, tact, and compassion in conflict resolution.
  • Maintain timely record-keeping.
  • Work effectively with diverse individuals and maintain positive working relationships.

Physical and Work Environment Requirements:

  • Must be physically capable of reaching, pushing, pulling, stooping, bending, standing, walking, and lifting up to 20 pounds.
  • Possess a valid Driver's License and maintain a safe driving record if required for job performance.

Work Environment:

The work environment may include outdoor conditions and off-site locations. The noise level is typically mild to moderate.

Job Hazards:

  • Work hours are structured but may require flexibility based on needs.
  • Position may involve tasks with potential exposure to infectious materials.
  • Participation in annual training on infection control and safety is mandatory.
  • New staff must complete training on managing threats and emergency situations.
  • Certification in Cardiopulmonary Resuscitation is required within one month of employment.
