Position Overview:

The Patient Care Coordinator is responsible for overseeing the care and services provided to a designated patient population throughout their healthcare journey. This role ensures the attainment of optimal quality, clinical, and financial outcomes while coordinating necessary services and resources for patients and their families. Additionally, the Care Manager takes on a leadership role within a multidisciplinary team.

Key Responsibilities:

1. Collaborate with physicians, healthcare providers, and the multidisciplinary team to discuss patient goals, care plans, and progress.

2. Develop, implement, and uphold work standards and procedures that align with organizational policies and regulatory requirements.

3. Stay informed about advancements in the field to recommend new services, products, and equipment.

4. Assess, develop, implement, and monitor comprehensive care plans through a collaborative multidisciplinary team approach involving patients and their families.

5. Regularly revise and adjust care plans based on ongoing assessments of patient conditions.

6. Assume accountability for the effectiveness of care plans and patient outcomes.

7. Evaluate the appropriateness of care levels, diagnostic testing, clinical procedures, and ensure thorough documentation of medical records.

8. Review patient admission data to determine the suitability of care levels in accordance with established criteria.

9. Develop, implement, and assess clinical pathways and their variances.

10. Monitor patient progress through clinical pathways.

11. Maintain continuous communication with patients, families, medical staff, caregivers, and third-party payers as necessary.

12. Assist patients and families with educational resources before admission, during their hospital stay, and post-discharge.

13. Ensure patients understand third-party payer guidelines and facilitate discharge planning as directed by physicians.

14. Foster a positive work environment and contribute to the overall team effort within the healthcare facility.

15. Assist in the collection and analysis of outcome data.

16. Perform additional duties as assigned.

Qualifications:

- BSN preferred with five years of experience in a hospital or community setting; or

- Completion of a BSN or related degree within five years; or

- RN with MSN or a degree in a related field with three years of experience; and

- Must meet all licensure and certification requirements in the state of practice; or

- Obtain certification from the Case Management Society of America.

Skills:

- Excellent oral and written communication abilities.

- Proficiency in applying advanced principles, concepts, and techniques relevant to the nursing profession.

- Strong organizational skills.

- Valid driver's license in the state of practice.

- Willingness to travel to patient residences, agencies, and network providers as required.

- Required to be cross-trained in Utilization Review, Quality Assurance, discharge planning, and infection control.
